Through a sophisticated stratification process, individuals are targeted for interventions based on acuity level and potential for impact.

OptumHealth’s Diabetes Program provides the information consumers need to:

• Achieve and maintain optimum blood glucose levels
• Manage co-morbidities related to diabetes, including hypertension, obesity, dyslipidemia and depression
• Prevent the onset of complications by receiving evidence-based care, including A1C tests and foot and eye exams
• Live life to its fullest by maintaining healthy lifestyles and adhering to recommended treatments and drug therapy regimens

Diabetes program interventions include:

• Comprehensive assessments by specialized Disease Management nurses to identify gaps in care and assist in creating individualized action plans. Plans include both short- and long-term goals to better manage diseases and reduce risks.
• Proactive outbound and responsive inbound nurse calls throughout the clinical intervention to help members achieve treatment goals
• Help scheduling physician appointments and accessing appropriate care, including psychosocial services and community resources
• Condition-appropriate medication management, drug-to-drug interaction information and adherence to medication regimens and physician treatment plans
• Management of risk factors and co-morbidities, including hypertension and depression.
• Educational opportunities to learn about:
  • Chronic conditions, self-management techniques, warning signs and use of evidence-based medicine to control and manage the progression of members’ diseases
• Cost-saving opportunities through use of mail-order pharmacy, lowest-tier medications and pill splitting where applicable
• Programs that promote healthy lifestyle choices
