If you are receiving care for a catastrophic or complex illness, or a serious injury/trauma or transplant, Care Flex Case Management is there to help you receive appropriate care and services.
A Care Flex nurse case manager works to ensure you are receiving services in the most appropriate inpatient setting. The nurse case manager may work with you, your family, your health care providers and facilities, as well as other health care services, vendors, or support groups, to coordinate a treatment plan that uses your health benefit dollars as efficiently as possible.
Care Flex matches the level and method of case management services to your particular needs. Your nurse case manager will help you and your family to coordinate and negotiate quality treatment all the way through your recovery.

Disease Management
The Disease Management program provide health coaching and education resources for members with the following chronic health conditions:
- Asthma
- Chronic obstructive pulmonary disease (COPD)
- Congestive heart failure (CHF)
- Coronary artery disease (heart disease)
- Diabetes
- Hypertension (high blood pressure)
- Depression
Disease Management RN condition coaches will reach out to you by phone or mail if you have a chronic health condition and can benefit from disease management services. You’ll receive phone sessions with an RN condition coach and useful educational materials for dealing with your chronic condition. Disease Management services can help improve your health and overall quality of life. It can help you learn self-management skills and make healthy lifestyle changes.

Maternity Management
Healthy women are more likely to have healthy babies. That’s why it is important to provide support to women before they become pregnant and throughout their pregnancy. You will receive educational information based on your specific needs. Pre-pregnancy coaching engages women when they are considering a future pregnancy. After enrollment, the member will work with a nurse case manager to complete a comprehensive assessment and identify potential risks or concerns to discuss with her doctor or other healthcare professional.

Health and Wellness
Members identified for participation in the health and wellness program are recruited for coaching with telephonic outreach, engagement letters and educational brochures. The Care Flex coaching staff are all certified health coaches and trained health professionals from a variety of backgrounds. Health coaches typically meet with you telephonically between five and eight times over a period of six months. Coaches work with you to set realistic personal health goals and provide guidance and encouragement along the way. You will also receive educational materials in the mail from your health coach that address relevant topics to your behavior change goals.
