Case Management: Targets complex or catastrophic cases; licensed, certified nurses serve as patient advocates and coordinate treatment plans designed to provide the right care at the right time.
Disease Management: Identifies members who are affected by the seven conditions (asthma, chronic obstructive pulmonary disease (COPD), coronary artery disease (CAD), diabetes, heart failure, hypertension and depression) that drive medical claims costs for employers; provides educational resources and one-on-one coaching to help members improve their well-being.
Maternity Management: Offers preconception counseling, prenatal education and follow-up support throughout the pregnancy from highly trained nurses, and provides case management for high-risk mothers.
Health & Wellness: Provides educational materials, action plans, one-on-one coaching and health screenings to help members make behavior changes and healthy choices, mitigating downstream health care costs.
