CORONARY ARTERY DISEASE

Our program empowers individuals, in collaboration with health care providers, to effectively manage their condition, and associated risk factors and co-morbidities.

OptumHealth’s Coronary Artery Disease (CAD) program is designed to reduce unnecessary hospitalizations and health care costs, and improve quality of life.

CAD program interventions include:

  • Comprehensive assessments by specialized DM nurses to identify gaps in care and assist in creating individualized action plans. Plans include both short- and long-term goals to better manage diseases and reduce risks.
  • Proactive outbound and responsive inbound nurse calls throughout the clinical intervention to help members achieve treatment goals.
  • Help scheduling physician appointments and accessing appropriate care, including psychosocial services and community resources.
  • Condition appropriate medication management, drug-to-drug interaction information and adherence to medication regimens and physician treatment plans.
  • Management of risk factors and co-morbidities including hypertension and depression.

The CAD program has delivered the following results:

  • 6.5% reduction in admissions
  • 1.6% reduction in inpatient days
  • 96% consumer satisfaction rate
