Case Management

Our case management services provide the education and expertise needed to help improve outcomes:

• Transitional Case Management evaluates and coordinates post-hospitalization needs for members identified as being at risk of rehospitalization or as frequent users of high-cost services. A recent study demonstrated 30 day readmits decreased from 35.85% to 12.66% through participation in this program.
• General Condition Management services are for those individuals with chronic conditions, those in need of longer-term support, or those who have unmet access, care plan, psycho-social or knowledge needs.
• Complex Case Management deals with catastrophic cases needing coordination of multiple providers, access to community resources or longer-term support or those having complex access, care plan, psychosocial or knowledge needs.
