Identification: To ensure that all the right people have access to the TDS program, eligible recipients are identified through multiple channels, including notification, health assessments, predictive modeling, provider data and referrals from other programs.
Engagement: We contact individuals at the right time—before treatment decisions are made—to increase the opportunity for positive behavior changes to make a difference.
Coaching: TDS nurses work with members to increase their participation in the care process. By assessing an individual’s understanding of their condition, their decision-making style and their urgency in moving forward with specific care decisions, the TDS nurse can educate, coach and guide individuals to make strong treatment decisions.
