Case Management

Our Case Management improves clinical outcomes for at-risk members while reducing overall costs and improving member satisfaction. Case Management improves care-seeking behaviors; ensures coordination for co-morbid behaviors and other conditions; and drives up-front, early interventions.

Our Case Management combines sophisticated technology with personal outreach to identify and support high-cost, at-risk members. With stringent criteria designed to ensure services for members who can benefit the most, the program:

  • Shifts utilization and supports healthy member behaviors
  • Provides a support system to facilitate treatment, information, and service
  • Coordinates and integrates across medical, behavioral, social and community services.

Interventions target high-risk/complex condition management, readmission management, and rare condition management to impact the quality and cost of care for public sector members.
