The COPD Disease Management Program Empowers Individuals
This program empowers individuals, in collaboration with physicians and other health care providers, to effectively manage their condition and associated risk factors and co-morbidities.

- Coordinated – taking advantage of our full continuum of wellness, health care decision support, case management and complex medical condition products and services
- Comprehensive – providing a whole-person approach to disease management that includes behavioral health, co-morbidity and prescription drug management
